How much apple cider vinegar per day?
15 to 30 ml a day. One to two tablespoons of liquid ACV diluted in at least 240 ml of water. Or two to three standard ACV gummies. This is the dose range tested in the published systematic reviews. Below this you cannot reasonably expect the documented benefits. Above this you add side-effect risk without adding measurable benefit.
Why 15 to 30 ml a day is the right answer
Every published systematic review and meta-analysis on ACV that found benefits used doses in this range. The dose-response data shows a clear plateau above 30 ml a day. The side-effect rate rises sharply at higher doses. Four points explain why the standard recommendation is firm and what happens when you deviate from it.
1. The clinical evidence sits at 15 to 30 ml
The 2025 PMC12472926 weight loss meta-analysis used 15 to 30 ml daily across the included trials. The 2025 Frontiers GRADE-assessed blood sugar review used similar doses. The 2022 PMID 36152934 blood pressure meta-analysis modelled dose-response up to 30 ml a day. The 2021 Hadi lipid review used the same range. Every benefit you can cite for ACV comes from this dose window. Lower doses produce too-small effects to reliably detect. Higher doses are largely untested.
2. Above 30 ml the curve flattens
The 2022 blood pressure dose-response meta-analysis is the clearest example. The effect per additional 30 ml plateaus. Doubling the dose does not double the benefit. The same pattern applies to weight loss, satiety and blood sugar effects. Acetic acid receptors and the gastric emptying mechanism both have saturation points. More ACV beyond saturation adds nothing measurable.
3. Above 30 ml the side effects rise sharply
The 2020 Launholt safety review (PMID 32170375) documents the side-effect pattern. Tooth enamel erosion rate rises non-linearly with dose. Throat irritation becomes common above two tablespoons in a single sitting. Stomach upset and diarrhoea risk increase. Potassium loss with long-term high-dose use becomes more pronounced. The risk-benefit ratio gets worse fast.
4. The case for splitting the dose
30 ml as a single dose has the highest peak acidity and the highest side-effect risk. 15 ml before lunch plus 15 ml before dinner spreads the load. The total daily acetic acid exposure is the same. The peak gastric acid load at any one time is halved. For people on the higher end of the dose range splitting is the smarter choice. For 15 ml total a single dose is fine.
How to dose ACV correctly in real life
Five rules cover the practical side. The studies that showed benefits used roughly this protocol. Deviate from it and the effect drops or disappears.
Start at 15 ml a day
One tablespoon diluted in 240 ml of water. Take it 15 to 30 minutes before your main meal. Use this dose for the first two weeks while your stomach adapts. Most people get the documented benefits at this dose. Moving up is optional not necessary.
Move to 30 ml only if needed
After two weeks at 15 ml if you tolerate it well and you want the upper end of the benefit range move to 30 ml total split into two doses (15 ml before lunch and 15 ml before dinner). Do not jump from 0 to 30 ml. The adaptation period prevents most digestive complaints.
Never exceed 30 ml a day for daily use
Above 30 ml the benefits plateau and side effects rise. The dose-response evidence is clear. The exception is short-term cooking use (salad dressings, recipes) which is incidental and does not count as supplemental dosing.
Dilute properly every single time
240 ml of water per tablespoon minimum. A large mug or standard tumbler. Drink through a wide straw to limit tooth contact. Rinse the mouth with plain water afterwards. Never drink ACV neat. Never as a shot. Never undiluted.
Time it 15 to 30 minutes before food
The satiety and gastric emptying effects work best before meals. ACV after eating produces minimal benefit. ACV between meals or first thing in the morning on a totally empty stomach increases the chance of digestive upset. The 15 to 30 minute pre-meal window is the documented protocol.

- Severe abdominal pain that does not ease after stopping ACV.

- Symptoms of low potassium such as muscle weakness, cramping or irregular heartbeat. Long-term high-dose ACV can lower potassium.
- Worsening of an existing condition such as gastritis, IBS, acid reflux or ulcers.
Anyone taking diabetes medication, diuretics, digoxin or blood thinners should also speak to their GP before starting daily ACV because the interaction risk is real even at standard doses. Pregnant or breastfeeding women should also seek advice before regular use.
